Diabetes Education

Diabetes is a chronic but manageable disease that affects 1 in 20 Australians.

At Community Care Chemist, we are proud to offer diabetes consultations with a Credentialled Diabetes Educator in-store at our Grovedale pharmacy and via telehealth across our other pharmacy locations.

Suitable for the newly diagnosed or people who’ve lived with diabetes all their life, our diabetes consultations can support you with advice on medication, basic nutrition and lifestyle factors.

Our Credentialed Diabetes Educator can:

  • Teach you more about diabetes
  • Help you monitor your blood glucose levels (including trailing continuous glucose monitoring)
  • Advise and support you to make lifestyle changes
  • Educate you on other health problems associated with dibetes and how to help you prevent them
  • Coach you on how to use medications, including injectable therapies
  • Help you access subsidised products and services you may need to manage your diabetes through the National Diabetes Services Scheme (NDSS) 
  • Link in with other members of your health care team, like your GP, endocrinologist, or allied health practitioners.

Standard diabetes consultation costs

30 mins – $90^*+

In-person at our Grovedale pharmacy or via telehealth at other locations.

^Medicare rebates may be available for eligible patients, learn more at Diabetes Victoria  

*Private health insurance rebates may be available, please contact your private health insurer to check if your level of insurance cover includes diabetes education prior to your diabetes consultation at Community Care Chemist. Some insurers do not include diabetes education in their products.

+Shorter or longer consultations are available at the discretion of the diabetes educator with varying prices. Please discuss your needs prior to booking.

How to book a diabetes consultation?

GPs and health practitioners can refer diabetes patients for a diabetes consultation directly to our educator at our Grovedale pharmacy via email: diabetes@communitycarechemist.com.au

Please include:

  • Most recent blood test results
  • Care plan referral
  • Any specific concerns or questions*

*Our diabetes educator is not able to assist with insulin pumps or pump concerns

National Diabetes Services Scheme Products

Community Care Chemist is a registered NDSS Access Point.

NDSS products (insulin pump consumables, needles, syringes, blood glucose and ketone strips and urine test strips) are available to order from Community Care Chemist pharmacies across Geelong.
