Medication Review

We understand that managing more than one type of medication or treatment can be challenging. That’s why our in-store pharmacists work collaboratively with you through our free MedsCheck and Diabetes MedsCheck medication review service to provide you with advice on your prescription and non-prescription medication, their safety when taken together, and how they work best for your health 

Who is a MedsCheck suitable for? 

You are eligible to receive a free MedsCheck medication review if: 

  • You are a current Medicare cardholder or Department of Veterans’ Affairs (DVA) cardholder 
  • Have not received a MedsCheck, Diabetes MedsCheck, Home Medicines Review (HMR) or Residential Medication Management Review (RMMR) in the previous 12 months 
  • You live at home in a community setting and; 
  • Take five or more prescription medicines or 
  • Have recently experienced a significant medical event. This may include a new diagnosis, changes in health status, or medical interventions or 
  • Take a medication associated with a high risk of adverse events. 
  • If you are unsure, check in with your local Community Care Chemist to see if you’d be eligible. 

What does a MedsCheck involve? 

Your MedsCheck takes place in-store in a professional consulting room to ensure your privacy. You and your Community Care Chemist pharmacist will have a face-to-face consultation where you can discuss your medications and any concerns you may have.  

Additionally, your pharmacist will review which medicines you are taking, and advise what each medicine does, and advise you about the best way to utilise and store your medicines. 

Who can benefit from a Diabetes MedsCheck medication review? 

A Diabetes MedsCheck is tailored specifically for people with type 2 diabetes and aims to: 

  • Improve your understanding of your diabetes medication 
  • Teach you how to use your blood glucose monitoring devices more effectively 
  • Help you manage your blood glucose levels 
  • Reduce the risk of developing complications associated with type 2 diabetes.
